Weeks 1–4: 1 mg per week
Weeks 5–8: 2 mg per week
Weeks 9–12: 4 mg per week
Weeks 12 plus, higher doses of 8mg and highest doses of 12mg may be achieved
For 10mg add 2.5 ml of saline Solution
